We are preparing to do the 12.2 upgrade and have discovered while working in the test environment that the new Family History and Social History forms do not behave properly, so badly in fact that it is unlikely that we will use them. For the Family History form, the only information that translates to the text is new values and if a review has been performed (our providers are accustomed to seeing the entire family history in the note). The established family history does not translate, and to my knowledge there is no way to manually insert this information.
The Social History form is even worse. Any time an update is made (smoking status, social behavior etc.) and the value is inserted, it does not carry the previous information from the comment (SH template box) forward so all of this information is lost. If we use this box to document marital status, occupation, children etc., this has to be done every time? Makes no sense. It also does not translate the information to the text unless new values are added.
I'm hoping someone on here will tell me I have it all wrong and that there is an easy fix for these problems. I'm wondering if anyone has experienced this same frustration??
Not only does it not carry forward but on the social history form you cannot copy/paste- this workflow does not work consistently and the workflow described in the SPR is to manually retype the information. I just got off a call about this issue. My physicians are going to be very displeased.
We are on CPS 12.2, and our histories carry forward from previous visits. They can also be inserted into the note by clicking the reviewed button. Social history has a button that says something like "insert selected values" if you don't want to do a review. Our workflow is to have the nurses review this when rooming the patients.
In the CCC form edit files, we have observation terms tied to Family Hx values. (Not sure if everyone is like this, but this is how we are set up for pediatrics.) We used the obs terms to create a form in our patient portal tied to the same observation terms. It is part of our registration process, so every patient has imported family hx already at the first visit. We just have to review it, but the entry is done for us by the parents.
I'm wondering if you need to add observation terms to your list of history choices (in the CCC edit file) to make the information pull forward for the next visit?

I work with Casey and I think we need to make a clarification. We agree that the old CCC forms for Family HX and Social Hx work fine (although the family hx form has always been very clunky). Casey is referring to the new HTML forms which were released with the 12.2.x version of CPS. Moving forward with HTML forms, you will have to customize your forms by manually changing the JSON files that are provided with the form. We have been able to customize the form for our needs; however we have now discovered that the form will not translate prior history to the note the way any reasonable person would expect. As Cheryl pointed out above, you have to re-enter all of the historical data that was previously captured in order to get it to appear in the note. This is a huge oversight by GE. If they want folks to migrate to the new HTML format, they should provide content that functions identically or better than the forms they have provided in the past. Regardless, I think we have figured out how to accommodate our needs, but it's going to require some ninja-like coding to get the form to behave the way it should (something we really shouldn't have to do). I really hope GE monitors these forums. Sorry for the rant. 🙂
I reported this issue to GE awhile back now. They were able to recreate it in their environment. The SPR is 68875 and the SR #1-992924307.

I have posted a Histories form to the Marketplace that supports documentation of family and social history if you are looking for an alternative. I will likely be posting a new version soon that also includes surgical and psychiatric history as well...
Were you able to alter the code to get the comment box to default prev SOCIAL HX? Like in the previous version?

That comment box is now opening as OBSPREV SOCIAL HX for us on the HTML form.

The form is working great now. If i put in anything in the comment box, it will stay there. I have it set up to where the comment box will default prev SOCIAL HX.
My issue now if that the old form's comment box saves that value in a weird format with hard returns and such. So when you pull it into the new HTML form, it doesn't pull it in correctly. You have to re-enter the old data in the comment box, then after that, the form works great again.
Has anyone attempted to alter the code?
