We are working on improving our ASCVD (Atherosclerotic Cardiovascular Disease) outcomes. We found on line ASCVD calculator form, but rather than just provide a link we would rather have the form available to our providers within CPS. Has anyone by chance already developed an ASCVD form in Centricity they would be willing to share?

I have a 10 yr risk calculator based on the 2013 ACC/AHA guidelines. Contact me and I will demonstrate it for you. It is currently within a lipid management form, but could easily be extracted for your use.
Here is the dlg of the calculator we use. It's pretty simple.

I have a clinical/technical question. Most of the ASCVD calculators that I have seen take 9 parameters:
Age, Sex, Race, Total Cholesterol, HDL, Systolic BP, Diabetes Status, Hypertensive Status, Smoking Status
However the website that most of the clinicians use is for the American College of Cardiology's calculator which takes an additional 3 parameters:
LDL, Statin Use, Aspirin Use

This has led to some confusion on my part in providing them a calculator in CPS. I've looked for a ASCVD formula with 12 parameters but Google has let me down. Can someone point me to a formula or otherwise explain the difference above?

I wrote a script that can be copied into a History View or Text Component - this pulls the data automatically from the EMR and performs the calculation without any user input. You may have to change some of the observation terms referenced in the script depending on where your cholesterol values map to.
