In EMR, if someone adds in a patient's problem into the list, but removes it without committing it to the chart, it is still showing up in the patient's problem list in Patient Portal. Is that by design or a bug? We have long known if a problem was entered in Error on a patient chart then corrected it is still visible in the inactive view on the Portal. Even though it is not at all visible in the EMR. We were surprised to find that happens even if the problem isn't commited.
