We noticed today when canceling a previously scheduled appointment for some users the appointment completely disappears from the system.
It does not show as a canceled appointment in the Schedule view. It also is missing from the Appointment History section of the Appointments tab in Registration.
For some users it works as expected - so it is not every single user that has this issue.
We upgraded to SP11 on December 11th, so we are thinking perhaps this problem may have been introduced then, but that is just a suspicion at this time.
Have any others seen this problem?

We have not seen that, have you checked the settings in administration > schedule > appointment cancel to ensure that the delete appointment drop down is not set to Yes for the cancel type you are using.
