I have seen on a few occasions where a form was imported into Network Training (such as a new/updated Vital Signs form) and then the "Prescriptions" form was completely deleted from the EMR. All document templates that referenced the "Prescriptions" form were then replaced by the new/updated Vital Signs form. In some cases the "Prescriptions" form would still be in the system, but in others it was completely removed and we had to re-import it in.
This happened in our production environment this morning which of course caused some issues. Luckily we caught it fast, however I was curious if this has ever happened to anyone else.

This is a known bug within CEMR. It normally only happens when lots of forms are being imported quickly by the end user.
I wish there was a fix in place. Key is to have backups of everything!!!
This has happened to me as well. Very frustrating since it causes issues for the end users when they start getting MEL garbage in their note. I couldn't figure out what had caused it with the form I had imported.
Good and Not good to know that it's a EMR bug!!
