Is anyone getting a "Invalid Document Format" on incoming TOC's? If so any problems reconciling the document?
We are currently using CEMR 9.8.13 and Kryptiq Patient Portal.

Are they coming from EPIC? We have had quite a few from EPIC that have failed and have had a case open for over a year with GE. Most of the issues found are actually on the EPIC side from what I understand. I think the hospital that sends them has fixed issues with upgrades/patches. The case has been worked by one of my coworkers so I do not know the specifics.

No this is happening with many different referring systems. The main system we are concentrating on right now is a system called EMD.
