3 times (out of 4000 office visits) with in the last 6 weeks we hove found signed documents with: [MLI_FORM:1827589971144490]

in the document text . It has occurred only in office visits and no one remembers anything about it so apparently there were no error messages. From looking at the document, I can tell that certain forms were removed from the update at some point but I can't figure out how. Someone could do it manually by mistake but usually they notice it right away and ask for help. Only 1 form is removed from all 3 updates but there are 1-2 other forms missing from each. So far I haven't been able to duplicate this. I've tried manually removing forms form the update and from the document template but can't reproduced the text. Any ideas on why this happened?
That's a placeholder for the form that should get replaced when the note is signed. I would guess that the office visit text looked fine up until the point that it was signed, much like the 'odometer' bug that many people on this forum experienced awhile ago. What version of Centricity on you currently using?
We are on CPS 12.2.2, and I checked our database and we do not have any instances of it.
We are on EMR 9.8.10.
We have been having this issue ongoing since we started using Centricity in 2012. Apparently it has been happening a lot longer than that. We have an open ticket with GE about it and they have produced a patch for us that so far doesn't seem to fix the issue. The good thing is that we actually have the developers ears on this issue.
I did a query on all document types since we started using Centricity (since 2000) and have found only 15 documents with the 'MLI_FORM' text and 4 of those are documents that we imported from another Centricity user. The first date I have for it is 2004 in one of those imported documents. It's weird that what we'd see (if we'd been looking) only sporadically over the last 17 or so years should show up 3 times in the last 6 weeks. I hope it doesn't become a habit.
