We have been having this problem a lot since SP11 also, and the Centricity Admin Toolbox works like a charm. We are on EMR Oracle.

Carolyn, have you checked to see if the clinical list changes from those documents you are fixing with the Admin Toolbox have their clinical list changes signed? For example, obs terms from that document are showing up in the flowsheet?

Ugh. The clinical lists changes and the obsterms from the documents that we unlock are NOT signed. Thanks for the heads up, I know we are working on resolving that issue too, but I did not understand that they are related.
