ISSUE: Forms and text translation disappear from document
ISSUE: Forms and text translation disappear from patient chart updates. When the behavior occurs the document will be completely blank. The forms disappear from the inserted pane and the chart text disappears as well. You can re-insert the form and the text will re-populate both the form fields and the text translation. Contribution logging does show where the forms and text were entered. This does not appear to be a tablespace issue. When the text reappears after the form are re-inserted the text will remain if the document is placed on-hold again. When the text disappears from Centricity Business-Centricity EMR client document it will also be missing from the same document in the EMR only client installed on the server. The alert logs and mlerrorlogs appear to be clean.

DETAILED STEPS TO REPRODUCE: (include ALL key clicks, data entry & patient)
1. Log into EMR as h Winston
2. Click Find Pt. and go to a test patient chart
3. Start an Update click New and select an Encounter type with forms
4. Enter data into the forms and then place the document on hold
5. Now open the update back up and the text translation and the list of forms on the left hand side of update under Inserted are missing. If you re-add the forms hey will populate again.
CURRENT BEHAVIOR: When you enter the update on some patients chart after the upgrade to 9.2.5 the forms along with the text translation sporadically disappear.
EXPECTED BEHAVIOR: The forms should still be there and the text translation in the chart update should be there.
WORKAROUND: If the user has noticed and not signed the update, then they can re-insert. This only works in the event they know which forms were in the update to begin with. If they have signed the update then all data is lost.
