We upgraded to EMR 9.8.10 a little over a week ago. Since then, very occasionally, one of our providers will open an encounter that had been put on hold and find that ALL of the forms that make up that encounter will have disappeared. I can believe that one form could have accidently been removed by this provider but not all of them. And something similar happened to me when I imported a form that I had changed. Yesterday evening I changed the form, imported it, updated the template and tested the encounter to make sure it worked. When users open the encounter today the form was missing from the template AND was missing completely from Centricity. Has anyone else experienced anything similar?
We've been experiencing this since the 9.8.3 so almost a full year now. Providers return to a empty OV after they've placed it on hold, like you said all the forms magically disappear. Did you reinsert the forms? The data is all still there which makes it weirder. Seems like another one of GE's challenges to solve because they never made mention of it before we upgraded nor have we heard of them acknowledging the issue.

ISSUE: Forms and text translation disappear from document
ISSUE: Forms and text translation disappear from patient chart updates. When the behavior occurs the document will be completely blank. The forms disappear from the inserted pane and the chart text disappears as well. You can re-insert the form and the text will re-populate both the form fields and the text translation. Contribution logging does show where the forms and text were entered. This does not appear to be a tablespace issue. When the text reappears after the form are re-inserted the text will remain if the document is placed on-hold again. When the text disappears from Centricity Business-Centricity EMR client document it will also be missing from the same document in the EMR only client installed on the server. The alert logs and mlerrorlogs appear to be clean.

DETAILED STEPS TO REPRODUCE: (include ALL key clicks, data entry & patient)
1. Log into EMR as h Winston
2. Click Find Pt. and go to a test patient chart
3. Start an Update click New and select an Encounter type with forms
4. Enter data into the forms and then place the document on hold
5. Now open the update back up and the text translation and the list of forms on the left hand side of update under Inserted are missing. If you re-add the forms hey will populate again.
CURRENT BEHAVIOR: When you enter the update on some patients chart after the upgrade to 9.2.5 the forms along with the text translation sporadically disappear.
EXPECTED BEHAVIOR: The forms should still be there and the text translation in the chart update should be there.
WORKAROUND: If the user has noticed and not signed the update, then they can re-insert. This only works in the event they know which forms were in the update to begin with. If they have signed the update then all data is lost.
Yep, that's definitely it.
We also have had this, since 9.8.3. Both issues although the imported form thing has been longer. We had a couple times where our form imported and overwrote an existing form in our system.... what a mess. We were told at that time it has been known to happen and usually associated with errors in the form build, although we couldn't find anything and when we re-imported it was fine... of course we had to remove all the wrong form components from the document templates and replace it with what should have been there.
The missing form components in an update seems to be fixed when user puts visit on hold, goes to another chart, then goes back in and re-opens documents. Sometimes that doesn't work and the entire document template has to be re-inserted. Our SR was 1564091960 and they added us to cisdb00060889 which is supposed to be the fix for forms falling out.
You can create the issue in EMR 9.8 by selecting the entire chart note.
If you hit the spacebar, you get the message popup "This selection contains protected text - Text Protection Error" ... BUT... if you hit [Backspace] key, deletes entire text of chart note and all the forms....
