We have a provider that was documenting a chart note on a patient. All of a sudden, the entire content of the note disappeared. If you view the clinical list changes, you can see all the obs terms that were updated, and all the visit content that changed, but the actual note is blank. Even the forms in the encounter, that normally show on the left of the chart note, are blank. Has anyone ever seen this ? We do not know what could have caused this, or how, if possible, to get the content back ? Any information would be helpful. The provider is getting restless and wants an answer. Thanks.

Seen that before (on 9.5, but not yet on 9.8).
Never could get a reason.
Open the note, right-click and Insert Document Template. Re-Add the document template appropriate for your visit and it will reload all the forms.
Like James said, you can just add the document template that you use for the encounter back in to pull the chart note back. We've seen this happen to a few physicians, and I know one of them did it back accidentally selecting all of the text and hitting Backspace on the keyboard. This will remove all the forms.
We saw it on CEMR 9.5 and have even had it happen in CPS12 as well.
If deleting the all of text removes the form, it is possible that a couple of inadvertent keystrokes that happen to activate shortcut key actions could be the culprit.
We've also seen this happen and assumed it was an insane refresh issue. We have also seen it when 2 people are in the document at the same time.
We reported this to GE but, can't reproduce it so there was no way to "prove" it was happening or to figure out how to fix it.

I LOVE this. Select all back-space..... who knew? I really find it hard to believe that it the culprit for all the times this has happened to us, we are still seeing this in 9.8. Wonder if GE knows that select all backspace or delete removes all form components? It should not do that. Select all and hitting any key gives you a pop-up warning about protected text, backspace and delete should.

Posting a reproducible way to get this error.
1) Open a new (test patient) encounter.
2) Select stuff into the chart note.
3) Select entire chart note so everything is highlighted.
4) Click the [Backspace] or [Delete] key.
** POOF - all the forms disappear from the encounter and the chart note is empty.
5) And adding back the form(s) does pull all the data back into the chart note.

We started seeing occasional instances of something similar back on 9.5 that has persisted on 9.8. However, we are having problems reproducing since it appears to only wipe out the text upon signing the document, so they don't know until it's done. The clinical lists remain, so we do or best to recreate the note. Unable to reproduce for GE, and only happens about once every few weeks with over 250 providers charting.
We have had it happen to a few providers in version 9.8. The solution that Jamesedward posted is the 'workaround'. James' response didn't explicitly state it, but re-inserting the document template not only inserts the missing forms but all the missing text as well.
