We implemented the new immunization form a week ago. We can enter vaccines on most patients
without any issues but there are a few exceptions thusfar. We have an SR logged
with GE but we haven't heard back from them, waiting on a db expert.
It seems to me that the form freezing and the system displays a crash error message is happening more on Peds patients and when entering the MMR vaccine. Has anyone else experienced this problem? PS - When testing on a test patient, no problem at all. Thanks

Check the patients immunization history view for MEL vomit, if its there and if you can open a immunization management form in an update, check each vaccine for a special character. Ours was a "@" that just blew the form up and crashed charts. They typically be in the comments section. GE was able to give us a script to run in the database to correct this. I suggest seeing if it's the "@".
Gail, We also have this problem happening quite frequently for pediatric patients. Doesn't seem to matter which vaccine. I have had multiple calls and webexes with GE about this. Providers are getting very frustrated since they cannot view patient Immunizations and have fear of over vaccinating patients due to the new vaccines not showing.
Below are the SPR's for both problems with the immunization. We have also had luck with opening an update and pulling in the Imm. Form and then going directly to the Imm. Hx View and everything works fine for that moment. But this has to be done each time the Imm. Hx View is accessed.

This could be one of two known defects, addressed in SPR 62073 and SPR 62538.
adaniel: I'm not sure how we find "symbols" from previous information. I have tried to reivew a few ot the patients with problems, but it's like looking for a needle in a haystack. Any suggestions on how to find these easily?
If you ran the data migration wizard it should be pretty easy. We ran a simple report:
Find: Patients
Where: FLU VAX (any entry) contains @
Then set some appointment dates if you need to limit it down.
After that I go into a selected patients chart, open the immun mgmt form look at my flu and find which one has an @ and remove it.
We had this happen on only patients that had immunizations documented under TD BOOST obs term. It was one of the standard obs terms that should have converted but had something wrong with mapping tool. It put a null value in the table when it converted it. It was fine with other patients that didn't have that vaccine. GE was able to help with a script but we were only working in test, never went to production and I'm not seeing all of her scripts saved on our test server. The latest conversion tool on the customer site is dated right around that time so not sure if it has latest fixes or not - they were working on changes and may give you something later than what is on the site if you ask. I will be revisiting soon, if I get it I will provide here.
Thanks for all of the help/comments. I tried removing the @ sign and GE also ran a script that was suppose to remove all symbols etc that could be causing the crash. It has been almost two weeks and the form is still crashing sporadically.
This form has been available for many months so it is hard to understand why this problem hasn't been corrected already by GE.
