Portal answers importing into CCC forms

I have been editing our new CCC forms for a little while and now we've starting having patients fill out thier paperwork online and then it imports into CPS 12.

I am having trouble with the ROS-CCC form - the patient enters the information on the portal, the option is there for my nurses to insert it information into the note BUT the information only flows into the TEXT side of our office visit note. If you look at the check boxes on the ROS-CCC form, they are all blank.

Has anyone had anything like this before and how did you fix it? Or any idea on how to fix it??
