As you probably know, you can't record immunizations if another encounter containing unsigned clinical list changes exists. As a result, I don't believe we are capturing all immunizations.
For example, the nurse gives the immunization, and fills out the form. If they encounter the "unsigned clinical list changes" error message, the immunization is not committed to the database. Someone would need to go into the document at a later time, and finish recording the immunization. This is not happening, at least not consistently.
I'm wondering if any other organizations have encountered this issue, and how you are handling it?

Our Providers have been trained to always have the "Sign clinical list changes" box checked when they are putting a document on hold. this way, unless the provider is currently charting the Nurse can fill out an immunization document.
