When printing claim forms, the diagnosis codes are printing on every other line (i.e. A,C,E instead of A.B.C) and on the cpt line it is listing them at 1.2.3
Anyone else having this issue?

Make sure you have defined the correct crystal report file within CPS.
- Go to Administration>Reports>Business Report Criteria
- Select Claims and click Modify
- Ensure that Claims Stored Procedure = CMS1500_V0212 and Claims Report File = CMS1500_V0212.rpt

We have the same issue here after upgrading to 12.2. I set the claim store procedure to CMS1500_v0212, and it did not resolve the issue.

Please verify that the RPT file that is specified in Business Report Criteria exists in that path that is specified. For example, I have mine in c:\program files (x86)\centricity practice solution\client\pmcwrpts. PMCWRPTS folder is a not a folder that is created by default during installation, but that is where I keep my custom crystal report files so they do not get overwritten during upgrade.
Also, another thing you can check is when you preview the HCFA and try to save it, it will default to a filename (if I remember correctly) - that will also tell you which crystal report file it is using.
