We are getting this error more frequently this year, providers cannot add any clinical list changes to some notes because there are unsigned clinical list changes, and everything is signed. I see there is a script to fix this. Is there a reason for this issue? Is there something that we can improve on the forms?

This may help. We were having the same issue for years. We had the script in place and it was not help . It took us awhile to figure out the workflow.
The provider has the box checked to sign clinical list changes, and checks the box to not prompt me again. If the provider does not put the note on hold, and just got to another chart, clinical list changes are not signed.
Workflow. Provider is in open chart note in Patient A. Provider clicks the chart desktop button and selects and opens Patient B chart note. This will lock the clinical list for Patient A since the chart note was never actually put on hold, and no clinical list were signed
