So with this new observation term based PMH, how do you go about keeping your problem list and the PMH in sync?
Say you add probably manually or through the CCC problems form. It you keep current problems in the PMH or of if you make the problem inactive/resolved, then do you have to remember to go add it to the past medical history??? does anyone think about these things at GE?
Our clinic pondered this same dilemma. In the end, we decided that the Inactive problem list it would be best to use our combined PMH obs term and the Inactive Problem list for a complete past medical Hx, and to not worry about making sure they match eachother.
here is our basic idea:
PMH: Used to document medical Hx of problems that were not diagnosed and treated at our clinic.
Inactive Problem List: Used to document Hx of problems that were diagnosed and treated by us.
It is not the prettiest solution when you have to go to 2 places for the PMH info, but it has not caused any significant problems for us (just extra clicking for staff).
I have not heard of anyone else using a different solution.
