Hi all, my first post!
Our organization is upgrading from 9.5 to 9.8 in early June and I was wondering about general issues and concerns those site's who have already upgrade have seen? I know of a few we are monitoring: Merge not working, the CCDA not being very good from a workflow standpoint, needing to use the CCC forms. Are they any others we should be preparing for? I've scanned the forums here and these seem to be all I've seen but I'd like to hear from anyone who has already gone Live.
Thanks,
Mike
We are on CPS which is the combo and when we put it into out test server all of the patients had their race selections erased. So I don't know what you are using for you PM side I am not familiar with the 9.8.
We have it up in TEST and have applied the 9.8 SP1 yesterday. We use a lot of custom forms so we're trying to integrate some CCC with those, which is difficult. The biggest problem is that the CQR functionality is not available for general release, so we are not able to fully test the mapping from our forms to the reports which is leaving everyone uncomfortable.
On a good note, the new proposed rule this morning for MU2 delay may give us some breathing room.
Because of concerns with the ability to implement software, train users and make workflow changes, it appears that there will be a delay in MU2.

Thank you all for the responses! We may also be pushing out our proposed production Live. All in all there are about 20 other projects we could be doing so this may not be such a bad thing even with all the time spent preparing. If we move forward with our Live I'll be sure to update any other issues we run into as a beware to others.
We upgraded last Monday from EMR 9.5 to EMR 9.8 and these were the issues we encountered:
1. The Entry button on the HPI form. It's producing data errors and MEL language.
2. When entering orders, you may receive a pop up stating that there is no auth. provider for one or more orders.
3. Phone notes producing problem list.
4. For Peds 13-18, the Bright Future forms are not listed on the HPI inserted forms column. They are still accessible by inserting the form itself. Also, if clicking the PE for for this age group, it's taking you to the adult PE form. If you click the next form button, it will take you to the age specific PE form for peds.
5. Also, on eRX's, some have been getting a box pop up asking to confirm the provider of the refill. At this time, you will need to verify it.
6. Referrals and imaging test orders are printing instead of faxing to service providers.
7. When creating letters or in the chart text, the bold, italicized and underline buttons are not working.
8. Risk Factors form for Alcohol. If using the 'Load Prior Value' button, it's not populating the previous values. Also, if the Alcohol section is checked, the seatbelt, exercise, etc. is listed under so it's hard to see they are different items.
9. Sometimes when in the refill button, none or partial meds are listed. When you go to the med list, they are all listed. You need to change the med in the med button and add the refill there. If you go back to the refill button after that, all the meds and the refill you just did will populate.
11. When deleting the FH from 9.5 in the general comments, once you click Ok, they populate back in the general comments.
13. If the problem does not have a start date listed, and you go to make a change, it defaults to a start date of 1/1/1960, even if the patient was born after that date.
14. When completing orders, some have encountered a pop up stating that the printer was unable to access (printer name).
15. Reported that some patients can't see any of their vaccines while in their Kryptiq portal account.
16. When entering FH, it's duplicating on the problem list.
17. FH that was removed from being added to the problem list is still being added.
18. Orders are not showing in the CPOE or anywhere in the chart note.
19. Slowness in adding orders.
21. When checking the drug interactions from the medication button, there are none listed. You need to close the form and use the Drug interactions button from the main text prior to ending the update.
22. Some of the ICD-10 folders are mislabeled. It may state the folder contains elbow/upper arm items, but once you go into that folder, it's leg items.
23. The Midmark EKG's/Spiro's can be opened, but not able to edit.
24. Medications are being duplicated on the med list when completing the drug overrides.
25. On certain patients, getting a "too much data for field" error when using the med button, you have to go to Rx refill button to work.
26. HPI form is only opening on initial visit.
27. If our custom list descriptor for dx code does not match the descriptor in the reference database, it gives an error to the user that the code you've chosen is obsolete, may cause billing error. Dx code is exactly the same. They have the ability to click through and use it anyway.
Thanks for posting this Betsy. On the issues you have with forms, are they custom forms or CCC forms?

Sure thing.
We were using CCC 8.3.5. We upgraded to CCC 8.3.8 and loaded the recommended CCC-Basic suite.

Thank you very much for posting all of that Betsy, it will definitely give us a few additional things to test. Which SP are/were you at if I may ask? We just received 3 in TEST and are still having some issues.

Hi,
We have just loaded SP 2 into our back-up environement, not pushed to production yet. It is recommended that all documents be signed off since they did some form changes (but doesn't say which forms) which is a big deal for our providers. We did not want to do that to them 2 weekends in a row. We would love to get our hands on SP3, so we can get that in testing and maybe just do one push to production. Is SP3 in GA yet?
Thanks, Betsy
Hi Betsy,
I do believe it is available now for all; we pushed for it hard last week and got it fresk off the presses as they say but I think it's GA. Good luck getting it in! The merge fix was a show stopper for us so we held off; we will be moving forward in early July.
Thanks,
Mike
From Betsy's list:
8. Risk Factors form for Alcohol. If using the 'Load Prior Value' button, it's not populating the previous values. Also, if the Alcohol section is checked, the seatbelt, exercise, etc. is listed under so it's hard to see they are different items.
Note that CCC V8.3.8 (released in 2013) stores the alcohol yes/no field as ETOH USE instead of the prior flowsheet value, ALCOHOL COMM. It's briefly mentioned in the Release Notes PDF and the Risk Factors text file. Since the migration was from 8.3.5, that's likely why your past values don't appear.
We hadn't seen ETOH USE get much activity in years, so we plan to convert all the ALCOHOL COMM to ETOH USE during our production go-live.
update ml.obs set hdid = 12882 where hdid = 300014...
Thanks, James! We didn't catch that in the release notes, but this makes sense. I appreciate you pointing that out 🙂
We upgraded to EMR 98 SP3 on Saturday. Our biggest problems right now are:
1) the EMR client occasionally crashes with the error "Exception ACCESS_VIOLATION (0xc0000005)." This has happened on about 5 different computers and several Citrix sessions.
2) LinkLogic will only run on one of the four computers on which we have installed it. The others blue screen when attempting to run it.
3) Several users have received the error "Cannot retrieve the SQL query" when attempting to print orders.
1.) we have been averaging about 33 crashes a day. We run mostly on Citrix and have 450 user licenses. We have sent crash logs to GE, but apparently the crash logs do not give them any insight into what is causing the problem.
3) I believe we solved the "Cannot retrieve the SQL query" by having our Citrix engineers install the ODBC on the citrix servers
