Does anyone out there have an ADL (acts of daily living) form to add to an office visit note? We are looking to add it as a tracking mechanism for insurance purposes. For example:
"How is pain affecting your life? Which ADL's can you not perform due to pain:"
-brush teeth/hair
-drive a car
-cooking
-climbing stairs
-make bed
-laundry

There is a form Geriatric Assesment-CCC that has Instrumental Activities of Daily Living. I have not looked in to customizing it but it might be possible.

It does have a txt file that can be edited to your specifications. CCCQE-User-Edit-Geri-Assessment-TFE.txt should be in your CCC folder.
