Just wondering if anyone ever created forms for the ASQ3 questionnaires? There are obsterms so I would think someone is using them in some fashion.

The form is copyrighted. I don't believe that the authors allow implementation and scoring of the form. If I'm wrong, delighted to help you.

As mentioned below by Dr. Fitzgerald, the content is copyrighted. Sometimes, if a medical office contacts the copyright holder directly, they will grant permission (often with restrictions) and allow conversion for use in an EMR. Sometimes they also charge a fee (license) and other times they simply insist on credit being included in the build. It all depends. The important part is to secure the written documentation to protect yourself and your practice.
Still, other times, the copyright holder will not release the content and prohibit unauthorized use and formats (to the point they aggressively pursue copyright violators). I know that FRAX is one instance and I seem to recall that Brookes Publishing (the copyright holder for Ages and Stages), is another.
It is important for all content developers to research the copyright status to avoid issues downstream. Just because one site secured permission, understand that it may not be transferable, so before one obtains and implements 'shared' copyrighted content for use locally, ensure one's own permission has been obtained and that the content follows the requirements of said permission (I've seen it change over time). It may become costly if one does not.
Should you be lucky enough to obtain permission, Dr. Fitzgerald is a fine choice if you need help developing the content.
