Does anyone know what we use for a formula in order for the Patient Banner to reflect whatever we type in the "phone type" spot in the PM side? If we put "mom" in the phone type box, our patient banner will just say "None". The standard Patient Banner that came in CPS does not do this. It will actually say "cell/mom" or "dad".....whatever we type there.

Do you want the text "mom" to appear with the phone number in the banner? If so, just add the text before PATIENT.___PHONE:
"MOM PHONE: " + PATIENT.____PHONE
That's off the top of my head, but something like that should work. I'm not sure which Patient Banner was provided by default, so I can't see the banner code itself, but have you looked at how the default banner displays that information?
