A provider did an append. It shows in the text of the original document, at the bottom, as signed. However, there is no plus symbol, with the actual append ( identified by the "A") with the original document. Has anyone experienced this, or might know how this could happen ?

This is such a crappy program that everyone in our practice would never consider it if we did it over.
The appends should be in the original note--like EPIC and othe programs allow. It does show an entry that it was modified by whom and when, and keeps the original file elsewhere. If you hit the wrong key on End Update, you are screwed.
There are many other ridiculous faults that should have been fixed before this program was submitted for sale. I was hopping the upgrade was a fix but it is not. We need to be harsh with GE, because being polite has not had any results. Beware, compare this program to others like EPIC, Allscripts and several others. The salesperson makes it look great, but we assumed it was a competent program. Once the sale was made, they could not care less. We had had it for over 1 year, and our productivity dropped 20% and we had more work, and the charts were not as good.
I manage our laboratory. We are reviewed every 2 years by COLA. We had an excellent review. The examiner was looking into INR monitoring. The notes are unintelligible. she could not tell what the results are how the dose was changed etc. She recommended to do the Centricity note, but use an extra note to give the actual details. It is to be reviewed at the next time. This should not be necessary.
The list of complaints--serious ones--is great. I would have expected GE to at least be tackling them, but so far--nothing. I know it can be done. My son in law does this for NIH all the time. Please email me at [email protected] if you want a list of the serious deficiencies of the software.
