I was wondering if anyone knew how to fix the following problem. I have one CCC form that I am trying to implement (Geriatric Assessment), but it gives the message UNAUTHORIZED USE OF FORM. All of my other CCC forms work including the editor forms except for this one. Any help would be greatly appreciated.

I think that is part of a module you have to purchase. We purchased the OB/Gyn Module some time back.
Now this is not part of CCC Basic, but rather the CCC Forms Installation (latest version CCC 9.4). The CCC installation includes forms from all modules but we only have access to the OB/Gyn forms since this is the only module we purchased. Geriatric forms definitely sound like a separate module.
