So we are still using a combination of custom forms, CCCv4, CCCv5, etc...
We are really looking to eliminate all that and just "re-platform" on CCC 9.0, because heck GE knows best, right?
Anyways, first thing when we started our testing, on the CCC-HPI form, I noticed the CHF button at the bottom takes you to CCC-HPI2, not to CHF-Q&E as you would expect.
I noticed in the release notes this form was removed.
I looked back and see that it was included in 8.3.8. Is that the way it works, a form just gets removed without replacing them and without giving a reason? Just decided it wasn't needed?
I also see in the release notes that you can still use old forms if you want so I suppose we could import it from an older version, but should I be using something that has been "deprecated".
Any thoughts? Any reason why some forms get removed, can everything that was on the CHF-Q&E form be covered using the HPI-CCC2 (speciatly specific HPI)?
Note this form only appears as part of the "Disease Management" form list at the bottom of the HPI form, for Internal medicine and family practice.
CHF Q&E is still in the form set. My button on the HPI also sends me to the wrong form. You can add it to the insert forms column on the right of the HPI to make it available to your providers.
It is clearly stated in the release notes as being removed in version 9.0.
I got a clean install of 9.0 from GE and it is not present in the forms.
It might be in your system still because you had installed a previous version of CCC before? Please confirm.

Your absolutely right it does clearly state that. Support has been removed and all links to it were amended to the the HPI-2. We still have it in our system as it was never removed. It is not a ccc txt file. It is a clinical kit that was imported in previous releases. Sorry for the confusion.
That is interesting that they removed it because one of the quality measures is looking for the CARDIAC EF obs term that is on that form. Tracks left ventricular ejection fraction.
