Here is what we are using. They are pretty specific to what our Health Coach wanted but may give you some ideas if nothing else. The CCM-Time Keeper form is used to document CCM time that doesn't directly involve the patient.
We also use additional forms for Risk Factors,PHQ-9 and Mini-Cog to complete our CCM encounter.
