Has anyone developed work flow for tacking time and then billing for Chronic Care Management time. I would be interested in learning about it.

We added a form with one of the fields being obs(Time_Spent). We have this field on other forms as well, like phone note. Our staff are documenting their time there so that reports can be run daily to get a list of patients with CCM time and total time. Its probably not the best workflow, but it is cost efficient. We just could not see getting a ROI for Chronic Care Management software due to we are a smaller practice.

His CCM going for everyone, and would anyone be able to answer some questions from a small family practice office? What is everyone using for a Care Plan template, and how are you getting it to the patient? How are you all tracking time these days?
