after service pack 4 we had problems with centricity.DLL crashing with all attempts at dictation on the local machine. This problem persisted in service pack , 6a.
an apparent fix is listed below.
Exit Dragon
Go to the following folder: (Hidden Folder)
C:\ProgramData\Nuance\NaturallySpeaking11
Or
C:\ProgramData\Nuance\NaturallySpeaking12 depending upon your version.
Find the nsapps.ini file and open it. Search for the string “emrter”. You will see a line of code that reads emrter=9. Change that to read emrter=-1.
(large file == use search)
Restart Dragon, and the problem should be resolved.
Seems to be working.

dare I ask how you came up with such a solution?
Combination of GE support --Alex and Dragon support forums at Know Brainer

As another data point we are testing the same on 12.0.10 Patch 4.
