Good Afternoon Everyone,
I wanted to see if anyone has an electronic form in CPS that they are using for Physical Therapy. Looking to streamline our workflow and make the form electronic to hopefully push to our patient portal as well.

We purchased our PT forms from ONE/Data Drivers almost 2 years ago. Our PT department loves the forms, they are easy to use and have everything you would want. Check them out at http://designateddatadrivers.com/
