I was recently made aware of a problem where intermittently a diagnosis code will be duplicated in a billing visit and then both diagnoses are associated with the related CPT codes but on separate lines thereby creating a situation of duplicate billing for the same CPT code.
Many of the examples I've been made aware of relate to immunizations. The Z23 code gets listed twice in billing - once with the description on upper and lower case and once with the description on all upper case, and these codes go into billing as diagnosis 1 and 2 (or 1 and 3, or 2 and 3). Then the CPT codes for each immunization documented are listed twice in the procedures.
We are on CPS12, Service Pack 11. All orders are entered in the Chart and then pushed to Billing on the PM side where the billing staff retrieves the charges.
Has anyone else seen this problem?
We have. We see it primarily on immunizations. According to GE it was fixed in SP11 (we are still on SP10), but it sounds like maybe not?
We sometimes see that the procedures then duplicate as well, even though it was only entered once on the chart side, it will be listed twice on the billing side.

We also see it primarily with immunizations but I was just given an example today with a different diagnosis. And the problem is exactly as you describe it in that the orders were only entered once on the chart side but show up duplicated in billing.
I have reported this to GE and will see what they say.
Can you tell me what you are using for your content/forms in the Chart?
We are using CCC CQIC content. There are 4 different organizations (and 4 different databases) with the same configuration on shared servers. Two of the organizations have this problem, one organization isn't having a problem at all, and one organization just started having the problem after an update to the content.
I'm just wondering if there is a correlation with the forms.
