Our ICD-10 committee is starting to address the issue of cleaning up patients' problem lists.
One question they had was how to deal with duplicate problem entries - same code/description - and each entry may have one or more assessments. They would prefer a single problem entry by code/description, but keep all assessements associated with it.
Does anyone know if it is possible to merge duplicate problems? And if so, how it would be done?
We are still on CPS 11.1
