We are a cardiologist practice and we are starting to report Cardiac Quality Care for Pinnacle. Does anyone know of a form that is used to document this already. We need to document Ejection Fraction and test type, Cad, CHF, AFIB or Aflutter, CHADS score, all contraindictaions, age plan was discussed. Really looking to see what anyone else is doing for this process or if we need to create our own form. Thanks for your help.

Our organization is also beginning the process with FigMD and Pinnacle. We will have the results of the gap analysis of our historical data in about 3 weeks from FigMD. I would love to reach out to you as well to see what workflows your practice has adopted to capture the necessary data.
