Does anyone host their Centricity from a remote location (not a hosting service), and if so what method do you use for the application deployment? (VDI, RDSH, RDS, other)
Have you experienced any issues with any of the deployment method, for example, USB redirection of peripherals using RDSH or maybe performance issues with VDI.
Second have you had any issues with obtaining support from GE using a “non-supported” deployment method?
We have seven sites so six are remote. We have a 20M link from each site to our main campus and deploy through RDS. WE do have a number of users on VDI - but we have had USB issues with those so do not use them in positions which require a USB device. Most (but not all) have thin clients in the exam rooms which connect with RDS. Most providers and nurses "steal" their RDS session between exam room and their office system, but we do have one site which carries laptops around. "Stealing" sessions by the way can create havoc with CPS printing, we use PrinterLogic for printer deployment which pretty much solves this problem.

Our CPS infrastructure is hosted at a colocation facility. We use MS RDS with currently ~35 active users per server instance (I just counted--it may be time to add another server to the farm).
We have VPN tunnels set up between the colo and each site, and a print server at the colo to handle all printing. We do not use USB printers. The print server is configured for IP printing with all printers, and we use GPOs to deploy printers.
We have no shared domain credentials (like exam1 or nurse1 for example), so session stealing (if I understand you correctly) is not possible. All users are only permitted one active session across the farm, so if a user logs in from another client, they will resume their only active session.
It's been awhile since I've been involved in any USB mapping across the RDS session. I don't believe we use it much.
Our local computers are on the same domain, so mapped drives and shares are accessible from any site. Therefore saving a file to X:\test makes the file available on the RDS farm as well.

Thank you. That user density is pretty high (at least according to the HW calculator that GE provides) :). Do you by any chance use the CQIC\CCC forms?
We use the CCC forms. We do not use CQIC forms.
The GE HW calculator is a pretty rough guide. The last time I looked at it I think it still specified bandwidth per user session. 64kbps per RDS user is ok to spec for 1-10 or so users, but it doesn't scale. If I have 1000 users, I don't need 64Mbps of bandwidth. I need perhaps 6-12Mbps.
I believe the HW guide is/was an excuse for GE to push the cause of a problem back on the customer, but I can't say that's actually ever happened to us -- mostly because GE support ignores us I think. 😉 We very rarely call GE support. We usually figure out and resolve any problems long before we get a call back.
