Is there anyone out there that has a Chronic Care Management form out there? We are just getting started with this program and don't want to reinvent the wheel.

I do not do VFE but hope this will help any that need a CCM baseline. 🙂

I ran across this old posting for CCM and am currently in need of some information on how others are managing their CCM patients. Would it be possible to get a copy of the form set that you produced, and have previously shared.
