A provider has reported the 'blue lock' staying on her notes, even after she checks the 'sign clinical lists' before putting the note on hold. She enters data into the forms but not directly into the flowsheet. She is a long-time user and the problem has just started to occur within the last week or two.
Does anyone have any ideas of why she is unable to get the clincal lists signed (she is sure the box is checked and I tried it too and got the same result) and/or how to troubleshoot her problem.
Her workaround for now is to sign the note, and then do an append if another user needs to work in the chart.
Did you change your securities?
There haven't been any changes to securities.
I checked with another CHC - they said 'this happens on occaision when the table data that stores the Doc ID is confused as a signed note. So it keeps the note or clinical changes as unsigned. Usually we can have our DBA manually set to a sign state or check if the note is in a strange “lock” mode. '
Our provider was able to sign the note and then do an Append to her note. This is the only time we have ever had this reported to us.
