A couple weeks back I created a template form, shared here, relative to Telemedicine visits. I asked for and received comments on things to consider.
So.... now contemplating a form to help us care for COVID patients.
What would be things to be included? Some I can think of, although still unsure details:
Date and result of last test
Emergency Dept visits:
Hospital Admissions:
Current housing situation:

Hi Joe,
I would think that the normal workflow for HPI would cover most of what you are asking about, with the exception of the date and result of the last test. Typical HPI collection should include inquiries about recent visits to the ER/Urgent Care as well as Hospitalization history. Unfortunately, due to the 'blob text' nature of most HPI's out there, this can be and often is easily forgotten.
As a guide, the following is what is typically taught in Medical School with regard to HPI, though like a lot of things, the structure is often 'abbreviated' to fit the immediate need especially in narrative approaches:
- Chief Concern/Onset
- Mechanism of Injury/Events leading to symptoms
- Symptom(s)/Associated Symptom(s)
- Aggravating/Relieving Factors
- Treatments (Locations/Facilities, Dates, Disposition, Effectiveness)
- - Self Treatments, Medication (OTC), Medication (Rx), Therapy, Procedures, DME/Splints
- Diagnostic Tests
- Consultations
- Current Status (Work/School/Sports restrictions/limitations)
- Misc. (i.e. home situation, transportation issues, security concerns)
- Overall Trend

I hope this helps guide you on your quest. Perhaps a quick text for the test date/result could be added to the HPI for easy inclusion (or button) under diagnostic tests?
Need to see the positive result and date of first positive test,
whether CDC or OTH (state lab vs other),
any known contacts,
family quarantined?,
who lives w patient - alone vs responsible adult/teen,
method of follow-up (telehealth vs in-person),
date of last follow-up contact,
any repeat Covid tests? - date of first negative,
first day without fever or symptoms,
date released from isolation,
date family released from quarantine,
date of CoVID antibody test and result (to assess immunity, particularly useful after vaccines/next year).
I would LOVE to have a copy of this form ASAP, if somebody has one. Hard to develop forms and see sick patients at the same time, but we're all in this thing together.
