Has anyone encountered any issues with the Coumadin Management Form? We are seeing that the Current Coumadin Dose does not list and the field is always blank. I have opened a ticket with GE to verify there is not a known issue with this. We are currently on 12.0.3.1383. Any help or comment would be greatly appreciated!

We are having a lot of issues with this form.
1. When in the form, it shuts GE down randomly.
2. The nurse will type the instructions into the box, but completely different text will appear in the note.
3. When printing the future INR, it will not print the lab the Biscom fax box will open randomly and put the order on Admin Hold.
Very frustrating. I also put in a ticket with GE.
PER GE: "I just wanted to let you know that that form is a very old form that is not part of the GE factory forms. It is not supported by GE any longer. You can still use that form if you want, however GE has not updated it and does not plan to. You might want to check the CHUG to see if someone out there has updated it, as the files were provided to customer sites to be edited".
So my reply to her was asking if there is a GE supported replacement and I have not heard back yet. So.....back to checking if someone else has one or we go through development and pay to have a custom form created. Let me know what you find!
Just to clarify - we are talking about the CPOE-Anticoagulation-CCC form, right? I wonder if there has been any follow up on this. I'm interested in having a Spanish language form of the Coumadin Instructions handout. I suspect I would get a similar answer from GE so I haven't submitted a ticket, but let me know if you have other information. Thanks, Harriett
There is a Coumadin Management Form as well. So different form, but it wouldn't hurt to inquire with GE about your form.
