Anyone out there utilizing a custom PPD form and willing to share? Due to GE not including it on the HTML immunization mgmt we will be using a custom form. I was hoping to get an example. We currently have it on a custom Immunization form and its basically minor documentation of the PPD procedure itself.
Anything would be great, thanks in advance.

There is the option of adding PPD under the "Unknown Vaccine Group Name" on the new immunization form until GE fixes the problem. This would allow you to avoid having to create a completely custom form.

GE pulled PPD out of immunizations since it isn't really an immunization. They did ship a PPD form out with the CCC Basic package that works to document the items. Hope that helps

We modified a form with VFE complete with handouts pulling in the 48 hour read time etc...If still in need of a form, please let me know and you can take a look.
