Hi. We have 38 specialities and they all want several custom problem lists attached to their Problems-CCC form, to assist in moving towards ICD-10 use with ease and accuracy. A couple of related questions have come up, and our group would appreciate feedback from other organizations that have (or have had in the past) considerable reliance on custom problem lists).
1. Approximately how many custom problem lists does your organization have?
2. If you have numerous custom problem lists, or have had in the past, have you noticed significant loss of system performance that could be attributed at least in part to those lists?
3. Any other input about this? We know the intent of the Smart List, but getting providers to get the correct ICD-10 codes onto their Smart Lists in the first place is a challenge.

We have probably 25-30 custom problem lists at the most. We've never considered them as a source of poor system performance, so I couldn't comment on that. But, we are pretty sure that the higher the ratio of active problems:patients = slower performance. A major challenge is getting clinicians to remove problems from the active list. For this reason, I advocate using custom lists with time-limits on as many problems as possible; things like sinusitis, pharyngitis, cough and the like. I would not put time limits on things like hypertension, copd and other chronic illnesses. I'm hoping this will help with the active problem bloat.

We are a multi-specialty group and we are in the process of developing one (1) custom list for each of our specialties (Dermatology, Pulmonary, Endocrine, etc.) Our providers do not like the Smart list and would rather default to a custom list conducive to their specialty especially with ICD-10 around the corner.
This is based from a report in our PM system on the top 150 or so diagnosis for that specialty. Then our coding manager will give us the ICD-10 code so we can create a custom list in Centricity. A monthly report will also be run in our PM system to see if any new diagnosis need to be added to the custom list.
