Data Display in text translation only upon updating other fields

I have multiple Data Display fields from Patient attribute in a form. These translate into the chart note even without updating other fields in the form. I would like to suppress their translation if the form has not been used and make them populate into the chart note only if other fields in the form have been updated. Can someone help? Thanks
