I have a nurse that worked for one provider for several years which everytime that she hits the PHQ9 form that it defaults her this one provider everytime which become troublesome for the nurse every visit she enters. I have checked the settings and also the preferences for both and find nothing unless I am overlooking something does anyone have any idea's they could share would be greatly appreciated.

Can you share the PHQ9 form, or the form definition that is causing this to happen?
I am not clear what you meant by 'she hits the PHQ9 form' - a button to start it, she types data into it, (she whacks the piece of paper with a bat?).
My two thoughts are that there could be programming behind a button (depending on type of form used) that is alerting the particular provider. Or maybe she has that provider as her default on routing. For example, as a provider I always have it route to Nursing if I add someone to route the document to - if I want it to go to someone else then I remove the Nursing user and add a different user. To change this default you can remove it and then click the box in bottom left "save as default." You may have thought of these...but just in case.
Thanks burnsh! I have checked both the setting for routing for the provider and also the user and don't see anything.
Joeg1962 its when she is in the office visit they are a blue link button that you click that then allows the form to pull up and be sent to the provider or who you choose to route the document too and for some odd reason its defaults it back to the one provider everytime which then she has to remove the provider and then add the provider she wants the document to route too for every office visit. This normally would be set as a default setting in Centricity per the users profile but its not on the user or the providers settings.
