Has anyone experienced issues with the Dementia Cognitive Assessment pulling correctly in to CQR??? We have NUMEROUS documented on the MMSE observation term where they just are not pulling into the met category, while other completed the same way are working perfectly!!!!
I understand that patients must have 2 visits in the reporting period and it must be completed and documented on the flow sheet. Do these have to accompany an office visit at the time of completion?

OK, and this is only a vague memory - but something about the way users are setup. Who signed the documents met vs. not met?
For the user, look at the top right "eligible professional" and also the "licensed or credentialed".
I sort of remember that having an impact on meeting measures.

I just checked myself since I have entered some and I am a licensed or credentialed.
Thank you for any ideas! We are just stuck at this point!
