Does anyone have any depression screening tools that qualify for CMS 2? The only form I find in GE is the PHQ9 form. We need something for adults and adolescents.

I found it under Depression Assessment (PHQ-9). . It doesn't use any of the obs term DEPSCREEN that is listed in the Quality Reporting guide tho.

Cecil, I didn't receive anything. There are 2 ll's and 2 tt's in my name. Sometimes that throws everyone. 🙂

The PHQ9 is for adults, and the PHQ-A for adolescents, but the PHQ-A is the same as the PHQ-9 but has 4 additional questions.

PHQ2-PHQ9 This form is combined and hits all obs terms for reporting
