We have documents that we will route to a providers desktop and they will pop off in a matter of minutes. They literally will not stay there. We understand there could be workflow issues, but we have tested this ourselves on our own desktops and the document just drops off. Therefore, we have imaging reports or other important documents that have the potential to fall through the cracks if we don't audit daily. Is anyone else having this issue? GE is working on getting log files and tracings, but I was curious if we were the only ones... CPS 12 service pack 4

We are having the same issue with the system logging out even while the providers are working on notes. Also, if the system times out and there is a document in process but not ended, the updated work disappears.
I am curios if we having the same issues. I am getting complaints from our users about missing notes. They swear that is was there and now it is not.

We are on EMR 9.8.5 and have also had random occurrences of the disappearing document problem. Unsigned but largely complete encounter notes appear blank after the provider goes back in to the On Hold document to finish and sign the note. If the document template is reinserted the encounter note reappears.
I am curious, as well. We are on CPS 11 and have similar reports of documentation going missing, and so far we haven't been able to do anything other than rule out common culprits.
Open a case with GE, what they have had us do is hand over SQL tracings from the users desktop that the documents have disappeared. So far we are still in limbo and have not heard back, but the more cases that are opened the more likely they are to look into it further.....
You might need to clear your Database Cache, you need to try running the ResetCache job found on your sql manager.
