Anyone using the cloud version of Dragon Medical in a Citrix environment? If so, how is the ongoing price/performance compared to a local version.
I have a provider who would normally land on Medical Practice Edition 2 (most others use V10), but since Dragon, Windows7 and Citrix don't play well together, I wanted to see if the cloud version is better/faster/cheaper. Cost is a big consideration.
EMR 9.8x, Windows7, Citrix XenApp6.5 (moving to 7.5 with 9.8.10). Not Clinically Speaking, just regular Dragon Medical.
The cloud version is subscription based so if the provider isn't using it you can simply deactivate the account with the administrator account so you can save money that way.
We too use a citrix environment and it hasn't been a cheap endeavor so far but it will eventually save us in transcription costs. So far we have replaced all the pod thin clients with PCs as they tend to handle the microphone drivers much better than the thin clients, there is much less lag in the transfer. We had to purchase a new Citrix server as having any more than 10 users on the same box lead to severe lag to the transcription. It wasn't enough to really see much more than a small spike in anything more than a single core on our one of our dual xeon e5-2640 processors but it is enough that it can take up to 10 seconds to process the data. Our bandwidth is not an issue either... so that was our solution.
We initially installed the dragon software locally however there is an issue with the templates when done so, the [ ] boxes do not function correctly and there is no mac support so we opted to install it on the citrix boxes. With it installed on the citrix box we now have the ability to run it on any device as long as the mic is picked up by the citrix session.
The software is very finicky. We have one provider who constantly gets kicked off so we are trying to find a solution to that. We have another who keeps losing his macros and his profile becomes corrupt but we are able to retrieve everything.
There is a SP from Microsoft which you need to ask for because if you don't, every time another user logs into the citrix session it temporarily stops audio transmission and CS freaks out.
We still aren't done with the initial setup and are only testing it with a few providers due to all these problems. I hear that there is another version coming out that is 100% cloud based and should resolve some of our problems... we shall see.

We use Dragon Medical Enterprise in a citrix environment. Works pretty good but we do run into problems regularly. Dragon Sync agent needing to be reinstalled and some lag sometimes like Scott said. Also we have steered away from macros since they don't always work or can break when things change in CPS. It was just too much time to keep them working. Price is expensive upfront but ongoing cost isn't to bad. Overall while frustrating for IT sometimes its worth it especially for the doctors who can't type quickly. Saves some of them hours per day.
Side question on Citrix 7.5? Is this actually supported by GE?
I'm not sure if it is actually GE supported but Clinically Speaking was at Centricity Live in Florida. I'm sure they paid a pretty penny to be a vendor there. I believe we are running Xenapp 6.5 if that makes sense (I don't handle the server aspect of things) but we are going to be performing an upgrade to it probably by the end of February.

Dragon Medical Enterprise is extremely expensive and covers an entire hospital. In situations where there are less than 25 end users individual boxes of DMPE2 are a much better way to go. DMPE2 also have full select and say text control, full macro support and less lag time. There is going to be a new version 100% cloud based but the cost will be 135.00 per month per user with a 3 year mandatory contract. That makes the new product mush less appealing than DMPE 2'S 1,599.99 one time out of pocket cost. We have also heard rumors of a DMPE 2.3 that will utilize a newer speech engine and offer full Windows 10 support.

What vendor trained your organization on Dragon? We are trying to find a vendor to give us in depth training on how to use Dragon in GE Centricity. We have basic knowledge of how Dragon works but would like a more extensive training specifically when using the EMR. Any information you can give me will be deeply appreciated.

We use Clinically Speaking for Dragon support and have been for about 10 years. They also did our initial setup and training. Very happy with them. http://clinicallyspeaking.net
I wouldn't say we are very happy with it but it is voice recognition and never perfect. We have been using the power mics and macros thus far. So far all of our partners have called it quits and went back to paying transcriptionists. Our transcription bill is roughly 300K per year and even knowing that, they are back to transcribing.
One provider gave it a month to the day with some good results but in the end it took him double the amount of time to dictate even with his macros. We have barely paid for the equipment we purchased to get this thing off the floor.
We are patiently awaiting upgrades to the software as it is very finicky. So much so that the last upgrade we installed killed connectivity between our mics and the citrix cession. We got word on Friday that there is a new update that should fix the problems with the previous update.
keeping my fingers crossed because its laboring and tedious to install just to uninstall and revert back after some unsuccessful testing.
