We've had several reports of entire notes going blank after being put on hold by the nurse and when the doc gets to it there's nothing there. The stranger thing is when viewing clinical list changes info is all there, but once again there is absolutely nothing in the updates. It's as if someone manually right clicked on each form and removed them entirely from the update.
Is anyone else experiencing this or something similar.

We've been seeing one form, CCC's CPOE, disappear, and you even still see the placeholder in the note but the form isn't there even though auditing shows it was inserted. But not the whole encounter.
Sounds similar, we finally got to see one that was started at 7:40 am today, in particular what we have as an IM Office Visit that contains 16 custom forms. CMA goes in does what she needs to puts it on hold, same with the RN, the when our MD goes into it it's completely blank, all 16 forms on the left gone, only thing left is the summary line. Of course GE would like to see it duplicated, but we can seem to make it happen again.

Very random and impossible to create on-demand. Happens only once (jinx!) out of many thousands, but usually in a mixed template (some forms CCC, some not).
For us, it mostly occurred with OB/GYN. Right-click>Insert Document Template is your best friend.

Thanks Jim, nice to know we're not alone, unfortunate, but now we can eliminate the issue being our forms. We may get one once a month, since this January. We have about 60 providers so one every month isn't a drastic thing, but still very frustrating.
I had a ticket with GE regarding this same behavior for an update where immunizations were 'lost', and we did a webex. (Fortunately, the immunizations had been fully recorded and actually went to our state registry, despite their having disappeared from the note's text translation.) I sent GE associated files, and then follow-up inquiries. The GE response was that the user must have removed the forms, but there was no record of them having done so. Two months later, I recieved an email from GE that the ticket had been closed, with no explanation.
We continue to experience this on a random basis, with both CCC and custom forms.
