We are on EMR 9.8. Just installed SP9. I was told that the Well Child-Age Specific-CCC form is no longer supported. The Entry-CCC form is supposed to replace this form. However, when we pull up the Entry form, it is completely blank. No content, and the dropdown box for specialty is empty. Does anyone know if there should be a file specifically for this forms' content ?
For example, User-Edit_Entry.txt or something similar, in the CCC or CCC-TFE folder ?
We have never used this form before, and I vaguely remember, I think when implementing 9.8, that there may have been one ? I cannot locate it on our system. Thanks.

ours is loaded in the CLINICFOLDERNAME within the Client folder. CCCQE-User-Edit-Expanded-Hx-1.txt
I think GE changed it from Entry-CCC to Expanded History Form-2-CCC (along with the TFE).
