We have a patient that has over 2000 documents in her chart, over 50 medications, and about 49 problems on her med list. When anyone goes into her chart it is extremely slow. Doesn’t matter if its thin or thick client. Any document that is opened we get an error and have to click multiple times to get through it. She is a very sick patient and is seen weekly. Providers are unable to get into her chart without being kicked out by GE or having to click over 30 times to get through the pop-up errors. I put a ticket in with GE. I have tried everything I can think of to fix it. Any ideas. I am struggling. I can’t even get into her chart to look at her meds. Kind of a patient safety issue.

I agree with Brian, that is what I was going to suggest. We have had success with it.
Showing less documents should help with chart access. Do you run the ARCHIVE_OBS job against your database to limit the number of obs terms loading with each chart? That may help with chart access. You might want see if you can just just do visits with her with a clinical list update and just directly update problems, meds and the flowsheet rather than trying to use forms. You probably have forms that are trying to read through the problems and medication lists and are running into overflow problems. Most places in EMR have a 2000 (occasional 4000) character limit and you may have things bumping up against that. For example, the CPOE form probably chokes trying to build a drop down list of her problems as the entire list is over 2000 (and possibly 4000) characters.
David and Brian are both right. That should help quite a bit but there will be a little slowness always with patients with than many meds and problems.
We have had the same problem with several charts. We opened a ticke with GE and they said that SP 12 will fix this. In the meantime, our doctor has been using the workflow mentioned above and documenting in a note then updating clinical lists as necessary.
