Does anyone have a form set for tracking administration of Botox for Neurology?
Does anyone have a form set for documenting an in-office EEG and/or EMG procedure?
Any suggestions or assistance you are willing to share would be fantastic.

I sent you via zip file some of our forms that our Neurolgists helped me build. Let me know if you didn't get it. Can't really figure out how to upload it. They were one of the first sites I took live so it has been a long time ago. Botox form is a little more recent.

I sent them, let me know if you didn't get them. I also included our PEX that the providers helped me to revise and build.

I would love to see how you have managed EMG data. We have been scanning in reports.
Also, We just added a movement disorders associate. Do you know of any forms for the UPDRS or DBS programming? We don't even have observation terms for the later to build a flowsheet if they exist.
