There have been 3 charts now that when going to sign or add anything into forms it freezes and then we are forced to reset CPS. Has anyone come across this? If so, is there a fix?

Have you tried running a MEL trace to see what's going on behind the scenes with the form?
You can turn tracing on by typing Ctrl + Alt + M to start (just prior to opening the form), and then Crtl + Alt + M again to stop the debug trace (just after the form locks up). By default, the trace file is placed in the following directory:
(C:\Users\[username]\AppData\Local\Centricity\Logs)
where [username] will be the username of the individual logged on to the computer. Name of the trace file will be: TraceMel.log
Make sure to turn the trace off after you get the log file. Trace files can grow quite large.
You might also check to see if the issue is isolated to a specific computer or certain patients. If it seems to be isolated to a specific computer, I would exit CPS and run the "Reset CPS" program a couple of times and then restart CPS to see if it happens again.
are these custom forms that were programmed incorrectly? If so you may need to use the CPS utility application to remove them.
