I am wondering what other specialists do for the global period. Currently we set a pop up note on the EMR side but are looking for a better notification for the schedulers.
The problem is that often the charge is not posted prior to the first post op visit so the schedulers do not know that the patient is in a global period.
We were thinking of a patient alert note but noticed that there is not an easy way to manage these via an expiration date. We know as soon as the charge is posted we can depend on that but until then we are looking for a better suggestion.

Hi Janet! I wanted to let you know that we are a multispecialty practice of Orthopaedic Surgeons. GLOBAL PERIODS are human nature with our practice. We have two places in CPS that we record the global periods to alert staff.
STEP 1: Our sx coordinators put the global dates and body part within the Appointments Tab, top left hand corner box called Patient Appointment Notes. For ex. LT SHLD 4/6/15-7/7/15. Anything written in this box is pulled into an appointment. When our reception staff opens/ modifys an appointment from the scheduling module- they will see a global period right away in the appointment window, under the Patient Appointment Notes line. This helps to identify which patient's to collect a copay from or not.
STEP 2: We have an IT specialist who created a form [document in EMR] for Global History. While the surgical coordinator books the surgery, they open the patient's chart in EMR and start a "New Document" and select the Global History Document. The sx coordinator has two options to select, "is this surgery?" or "is this a fracture?" When one is selected, let's say sx for example, the appropriate drop down boxes appear to enter Surgeon, DOS, Global end period, body part and procedure. When the sx coordinator ends the update and signs this document, it saves to the patient's chart history flowsheet AND pulls up into the EMR PATIENT BANNER. So when our providers are working out of EMR, they will see any global periods up in the banner.

A question about the patient appointment notes, if the patient has later procedures do you remove the old note and add the latest one, or do you keep adding to the note?
As for the EMR side, if your patient has multiple procedures how do you determine which goes into the banner? Do you have some date logic or do you just pull the latest obs date. Now that I have asked that you probably pull most recent value from that obs term.

If the previous global entered in Patient Appointment Notes has passed its end date, then it is removed and the new one is entered. If a patient is within two globals for two different body parts, then we add on the new one on the next line.
For EMR, I used the list_obs function to pull all observations that were saved with our Global Histories form. Then I pull out the end date and if the end date < todaysdate, then it won't appear on the banner. Otherwise, it will list all globals as such:
Right Shoulder 1/1/15-4/30/15, Left Knee 2/1/15-5/31/15, etc.
Once the end date passes, the global automatically drops off the banner so there is no manual clean-up on that end. This way we can see all active global periods. The surgical coordinators will enter this information only AFTER the surgery has been confirmed to take place (which is confirmed within 24 hours of the surgery date).

We have gone into our Procedure Fee Schedule and entered the GPDays field for our procedures if applicable. Once you have your global period for a procedure set, a warning will display for any patient that the procedure code is used for. The warning displays if you are scheduling an appointment or posting a charge to a visit for that patient during the global period. Our schedulers will make a note in the appointment notes field if they get the warning display. Of course this will only work if a charge has be posted using the procedure code.
