Our Pediatrician has asked if there is a way to insert the Growth Chart Percentiles into her dictation. Does anyone have a way to add this documentation to an encounter?

We use the Vital Signs - Pediatric form, and there is a box (bottom left) that we check that will add the percentiles to the note. We are CPS 12.2 but have been using the form for a long time. Are you pediatrics only... or multi-specialty?

We are multi-specialty with a Pediatrics dept. We are currently using the Vital Signs-CCC form which doesn't include this. Would you be able to share your Vital signs form?

Is this form titled Vital Signs-Pediatric?? If so could you share that form with us. The only one we have is Vital Signs-Peds-modified and I do not see any buttons for Growth percentiles.

The form is called Vitals Pediatric-CCC. I believe it was in in ccc1.4v and updated in ccc1.5v. We are in the LA programs. I believe you will need to be on CPS 12.1 or higher to used the ccc1.4v and 1.5v forms. I am not sure o the EMR version.

What is the difference between the weight percentile on the growth chart and the weight percentile on the Vital Sign Pediatric Form. One of our pediatricians wanted to know why the weight percentile is different in the growth chart. For nutritional counseling she was not sure which value to use.

There is no difference for us with what is in the growth chart and what is in the form. We have the same numbers. I know we do have "pgd" files that are installed on the server, but I'm not sure where. I'm not IT, but I do remember supplying the files to our vendor. I can send you those files - maybe that is the difference?

Were you able to find out why you are seeing different values? One of our providers pointed this out as well.
